Methodology

Beckman Coulter DxI® 600 Immunoassay System

Includes AFP, hCG, uE3, inhibin A, and interpretation.

This test is used for evaluation of open neural tube (OSB) defects, Down syndrome, and trisomy 18.

Specimen Requirements

Draw blood in a gold-top serum gel tube(s). Spin down and send 1 mL of serum refrigerated.

Note:

1. A speckled-top serum gel tube (s) or a plain, red-top tube(s) is also acceptable..

2. The following information is required for processing:

A. Date of draw

B. Maternal date of birth (not age), weight, race, and number of fetuses in this pregnancy

C. Is patient an insulin-dependent diabetic?

D. Is this a repeat specimen?

E. Does patient have a family history of neural tube defects? If yes, specify.

F. Gestational age calculation (provide at least 1 of the following):

1. Date of ultrasound and gestational age by ultrasound on date ultrasound was performed. (Do not extrapolate to draw date.)

2. First day of last menstrual period (LMP)

3. Gestational age by physical examination

4. Expected date of delivery (EDD)

 

3. Gestational age on day specimen is drawn must be between 15 and 20 weeks; 16 to 18 weeks is preferred.

Reference Values

NEURAL TUBE DEFECTS

Patient results are expressed as multiples of median (MoM).

Results less than MoM cutoffs below are associated with decreased risk of open neural tube defects. Results greater than MoM cutoff are associated with abnormal and increasing risk.

DOWN SYNDROME

Results are expressed as relative midtrimester risk of Down syndrome with a cutoff of 1 in 296. A lower risk than 1 in 296 (eg, 1 in 500) is associated with decreased risk of Down syndrome. A higher risk than 1 in 296 (eg, 1 in 200) is associated with abnormal and increasing risk.

TRISOMY 18

Results are expressed as relative midtrimester risk of trisomy 18 with a cutoff of 1 in 100. A higher risk than 1 in 100 (eg, 1 in 50) is associated with abnormal and increasing risk of trisomy 18. A lower risk than 1 in 100 is considered normal.

 

Abnormal results are called back to the ordering physician.
