Specimen Requirements

1. Send isolated bacteria on appropriate agar or slant. Organism must be in pure culture, actively growing.

2. Label agar/slant with patient’s name (first and last), date and actual time of collection, and type of specimen.

3. Forward promptly at ambient temperature only.

Note:  Specimen source is required on request form for processing.
