Methodology

Roche Diagnostics, 5th Generation high sensitive cardiac troponin T assay.  (hs-cTnT)Electrochemiluminescence Immunoassay

In patients with elevated troponin levels or chest pain of less than 3 hours duration, serial testing is necessary to rule out acute events. The recommended interval for serial testing is 0 (baseline) and 3 hours. A delta troponin value will be included on serial troponin results between 3 and 12 hours apart. In collaboration with the OhioHealth Heart and Vascular Services Clinical Guidance Council, the laboratory has developed algorithms using data from our own patient population. Interpretive comments based on these algorithms will be part of the results.

Specimen Requirements

 

Plasma Only MINT GREEN TUBE

Mint Green (Li-heparin) plasma only. or Li Heparin green top tube no gel.  (Hemolyzed specimen is not acceptable.) Spin down and send 1 mL of heparinized plasma refrigerated.

Note:  1. Patients receiving therapy with mouse monoclonal antibodies may have erroneous results.

Due to interference, patients receiving high doses of biotin (>5 mg/day) should wait at least 8 hours after dose before having blood drawn.

Reference Values

Female <6 - 14 ng/L

Male <6 - 22 ng/L

  • A comment will be added to Troponin T orders on patients registered as Unknown for gender at the time of testing. The result will flag as critical >14 ng/L. A comment will display for the reference range: Patient sex not reported at the time of testing. Ref Range: Female ≤ 14; Male ≤22. 

 

Delta Troponin T (Abnormal indicates a clinically significant change in troponin concentration)

≤7 ng/L if initial troponin ≤ 35 ng/L

< 20% if initial troponin >35 ng/L
