Methodology

TEG® 6s Hemostasis Analyzer records the kinetic changes in a sample of anticoagulated whole blood as the sample clots, retracts and/or lyses in real time.

 

Indicated for monitoring a patient's coagulability and to guide product administration for Massive Transfusion Protocols.

Specimen Requirements

Specimen Type:

Whole Blood Sodium Citrate

 

Specimen Required:

Container/Tube: 1 Blue Top tube: 3 ml (preferred) or 2 ml

 

Specimen Volume: Tubes must be filled to arrow to ensure appropriate anticoagulant to blood ratio.

 

Collection instructions:

  1. Prepare the arm for normal phlebotomy using alcohol sponge and sterile gauze. Apply tourniquet and make a clean venipuncture using a 19 - 21G butterfly needle. Discard the first 3-5 ml, which contains tissue contaminants (can be used for other, non-coagulation testing). Draw two blue-capped tubes containing 0.5 ml of 3.2% (0.105 M) sodium citrate (pH7.4). Cap and mix blood with the citrate by gentle inversion 5-7 times in order to mix well but not activate clotting factors. Do not mix again until ready to assay.
  2. Collection of citrated whole blood from indwelling venous blood line.
    • Do not obtain blood from a heparinized access line, lock, or indwelling heparin lock.
    • Discontinue fluid drip, if required.
    • Draw 10 cc of blood with one syringe – discard.
    • Draw two blue-capped tubes containing 0.5 ml of 3.2% (0.105 M) sodium citrate (pH 7.4). Cap and mix blood with the citrate by gentle inversion 5-7 times in order to mix well but not activate clotting factors. Do not mix again until ready to assay.

 

Samples can be used after a 10-minute incubation (laying flat on their side) and within 2 hours of draw to maintain in-use stability

Specimen Stability

TEG testing must be completed by the following times after collection: 2 hours (RMH, GMC and MH)

CK-LY30= 0.0-2.6 %

CRT-MA= 52-70 mm

CFF-MA= 15-32 mm

Limitations: TEG 6s analyzer results should always be considered within the clinical context of the individual patient’s case. In the event of inconsistencies with the patient’s clinical status, samples should be repeated or supplemented with additional clinical information and/or data including performance verification in the user’s patient population.
